The Adult Family Living Program (AFL) is a provided by the Connecticut Department of Social Services. The program provides the support needed to age-in-place with the assistance of a principle caregiver that lives in the home. The caregiver can be a relative of the care recipient as long as they are not a legally liable relative(i.e. spouse).
To be eligible applicants must meet certain criteria, such as being a Connecticut resident, be at risk of nursing home placement, as well as meet certain financial criteria. Adult Family living is defined as personal care and support services that are provided to participants who reside in a private home by a principle caregiver that lives in the home.
There are four different levels of care that depend on the complexity of care needed. When a referral is made to DSS, someone will come to yours home for an assessment and help determine eligibility and level.
The AFL caregiver receives a non-taxable stipend based upon the level of care needed.

Supervises, feeds and dresses clients who, by reason of advanced age, physical disability or mental deficiency, need supervision. Also assists clients with other activities that are related to the clients' independent living and which cannot be performed by the clients alone due to health and or age limitations. Our goal is to help individuals and their loved ones live happy and fulfilling lives, all while ensuring they can remain as independent as possible.
A Homemaker provides for the maintenance of a safe and clean environment. Performs various housekeeping tasks, meal preparation, activities, and companion care services for clients in the comfort of their own homes. Our experienced and highly trained caregivers provide the highest quality of non-medical in-home care to both clients and their families.
Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide one-on-one attention and care that cannot compare in other settings. Our caring and compassionate companion care services include accompanying clients on walks, appointments, and trips to social/ recreational activities.

Jaimie is a registered Nurse with a Bachelor's degree from Southern Connecticut State University. She has worked as a Nurse in Hospital, clinic, rehab and home care settings and has enjoyed every experience as its own. While in nursing school she has worked as a PCA/CNA and understands the hard work and dedication it takes. She strives to find the best caregivers as she knows what it truly takes.

Jennifer has been in the corporate world since graduating with a Bachelors of Science in Accounting from Central Connecticut State University. She has been a Staff Accountant for over 8 years and prides herself on being efficient with attention to details. While studying she previously worked for a home care provider agency and learned first hand the importance of having quality and trusted home care.
